The Confirmation team welcomes you to this commitment to conversion.  Our Catholic Faith is such a gift!  We hope to share more information with you during this Immediate Preparation Time.  The following documents are available for those preparing for this wonderful Sacrament:

Retreat Form 1: ARCHDIOCESE MEDICAL INFORMATION AND CONSENT FORM -  The Confirmation Retreat is Saturday, September 20, 2008.  This form needs to be turned in no later than Thursday, September 18 at 2:00 p.m.

Retreat Form 2:  ARCHDIOCESE RETREAT LIABILITY FORM - The Confirmation Retreat is Saturday, September 20, 2008.  This form needs to be turned in no later than Thursday, September 18 at 2:00 p.m.

Confirmation Class Schedule for 2008 - Please note that each of the listed dates are mandatory.

Confirmation Sponsor Form - This form is designed to be filled out by your sponsor and the Catholic church that they attend.  We will have a class that speaks about choosing a sponsor.

Confirmation Saints Project - This is due on Tuesday, September 23rd, 2008.

Service Project - This project is to be done over the Summer of 2008.  A reflection paper will be due on Tuesday, October 7, 2008 on your experience over the summer. 

Service Hour Form - Many places ask for a form to fill out after you have completed service at their establishment.  You may provide them with this form.

Confirmation Service Hour Suggestions - This is a form that shows some suggestions from last year’s Confirmation Candidates.

Confirmation Ledger Form – This form is used to record the Sacrament of Confirmation.
       
We will go over everything in more detail at the Parent & Student Confirmation Orientation Session on Tuesday, August 19th, 2008 at 7:00pm in the Chotin Center. 

• Adults:  If you are an Adult who has not been Confirmed, please refer to the Rite of Christian Initiation of Adults (RCIA) section. 
• Teenagers:  if you have missed the Confirmation program in your school, please contact us so that we can help you receive this great grace given to us by the Lord.  

We work throughout the year with the Rite of Christian Initiation of Adults (RCIA) which helps both Adults and Teens seeking Confirmation to receive this Sacrament. Adult Confirmation is held every year on the Solemnity of Pentecost, at the Cathedral Basilica of Saint Louis in New Orleans, with the Archbishop presiding.
